Form - Introduction

With the Form app, you can easily create forms and surveys to collect feedback from other users on a variety of topics.
Design polls or questionnaires to gather opinions from other members of the ENT platform.

This module allows you to build forms with a variety of question types:

• Single choice (dropdown menu or radio button)

• Multiple choice

• Rating scale

• Date selection

• Time selection

• Short text answer

• Long text answer
  … and more.

Thumbnails

On each form’s thumbnail, icons help you quickly identify your permissions and actions related to the content:

1- The form has been shared in response mode with users.

2- You have shared the form in management or contribution mode, or it has been shared with you in those modes.

3- A reminder to respond has been sent to users you shared the form with in response mode.
